Terms & Condition :By clicking & downloading the KAC Girls Registration Form PDF file (below) you agree to the terms & conditions as set forth by KAC Basketball that changes and or modification to the registration form is NOT allowed. If any modifications to the KAC Basketball Registration Form is found the registration form in question will be destroyed upon receipt and the applicant will be banned from registering in the KAC Basketball League.

REGISTRATION FORM INSTRUCTIONS
Registration Form, you will need to fill out & print both pages of the registration form. Read all pages carefully before signing.
- ✴Print, sign & date towards the bottom of page 1, just above the “For KAC use” line.
- ✴Page 2: PARENTS CONTRACT DISCIPLINARY ACTION Print, sign & date towards the bottom.
- ✴Please print pages 1 & 2 on 1 sheet of paper, front & back.
After filling out the registration form bring it with you on registration day with your child’s birth certificate & payment. The date of registration can be found on the Registration Notice on our homepage, under “Important dates”.
